New Service
To begin new service, come by our office at 5631 Waterlevel Highway, Cleveland, TN 37323. We will need proof of ownership or lease agreement (depending on your situation) and a copy of your government issued picture ID. An application fee will be payable to begin service, and the fees can be found in section C, Administrative fees, in our rate schedule.
If you are the property owner, please bring proof of ownership such as:
- Warranty deed
- Tax document
- Building permit for new construction
If you are renting, please bring your lease agreement. The agreement should include:
- Lanlord’s name and signature
- Lessee’s name and signature
- Name of all adults living at the property
- Date that residence started

To facilitate the process of signing up for a new account, a copy of our contract is available here for download. Please print the contract, fill in your personal information, and bring it to our office along with your government issued picture ID and proof of ownership if you own the property or lease agreement if you rent. An application fee will be payable at that time (see rate schedule for current fees).
The application fees for owners and non-owners (renters) are found at the top of section C, Administrative Fees.
